      <P>A cover letter</A> should accompany the manuscript. It should =

      the name, mailing address, telephone, fax number, and e-mail =

      the corresponding author. <B>An e-mail address is essential.</B> =

      cover letter, indicate the category of manuscript (Clinical Study, =
Review,=20
      Special Article, Brief Observation, Letter). Suggestions for =
potential=20
      reviewers are welcome.</P>
      <P>The cover letter must attest that the manuscript represents =
original=20
      work, and that it is not under consideration for publication =
elsewhere.=20
      The cover letter should also state that all authors meet criteria =
for=20
      authorship and that the authors will sign a statement attesting to =

      authorship, disclosing all potential conflicts of interest (see =
below),=20
      and releasing the copyright should the manuscript be accepted for=20
      publication. An electronic cover letter is acceptable for =
manuscripts=20
      submitted via e-mail.</P>
      <P>Each author must have participated in the writing of the =
manuscript,=20
      and have seen and approved the submitted version. Each author =
should have=20
      also been involved in the conception and design of the study, or =
the=20
      analysis of the data. We do not allow group authors (e.g., for the =
ACRONYM=20
      investigators).</P>
      <P>Authors of research and review articles, and editorials, should =

      disclose any affiliation with any organization with a financial =
interest,=20
      direct or indirect, in the subject matter or materials discussed =
in the=20
      manuscript (e.g., consultancies, employment, expert testimony, =
honoraria,=20
      speakers bureaus, retainers, stock options, or ownership) that may =
affect=20
      the conduct or reporting of the work submitted. If uncertain as to =
what=20
      might be considered a potential conflict of interest, authors =
should err=20
      on the side of full disclosure. Because reviews and editorials are =
based=20
      on selection and interpretation of the literature, the Journal =
expects=20
      that authors of such articles will not have any financial interest =
in a=20
      company (or its competitor) that makes a product discussed in the =
article.=20
      Information about potential conflict of interest will be made =
available to=20
      reviewers and will be published with the manuscript at the =

      <P>The Journal does not accept review articles that were sponsored =

      <P></P>
      <P>Requirements </A>for manuscripts are in accord with the Uniform =

      Requirements for Manuscripts Submitted to Biomedical Journals. For =
more=20
      information, refer to the following: International Committee of =
Medical=20
      Journal Editors. Uniform requirements for manuscripts submitted to =

      biomedical journals. Ann Intern Med. 1997; 126:36---47.</P>
      <P><B>Clinical Studies</B> and <B>Special Articles</B> should be =
no longer=20
      than 3000 words, from introduction through discussion. <B>Brief=20
      Observations</B> should be limited to 1500 words. The journal does =
not=20
      normally publish case reports as Brief Observations unless they =
provide=20
      important new insights into the pathophysiology or other aspects =
of a=20
      disease; however, brief case reports may be appropriate as a =
Letter to the=20
      Editor. <B>Review articles</B> must not exceed 4000 words, with no =
more=20
      than 100 references. The Journal also welcomes <B>Letters to the=20
      Editor</B> provided they do not contain material that has been =
submitted=20
      or published elsewhere. Letters must not exceed 500 words, with no =
more=20
      than 10 references and one figure or table. An electronic version =
as an=20
      e-mail attachment (preferably in Microsoft Word) must be submitted =
along=20
      with the paper version. The letter should be signed by no more =
than 3=20
      authors, with only one affiliation for each. Letters referring to =
a=20
      published Journal article must be received within two months of =
its=20
      publication. All materials submitted to the Journal are subject to =
editing=20
      and abridgment.</P>
      <P>Submit five copies of all elements: cover letter, text, =
references,=20
      legends, tables, and figures. An electronic copy on disk should =
accompany=20
      mail submissions. </P>
      <P>Only a single copy, with a cover letter, need be submitted=20
      electronically. E-mail submissions must be formatted for US Letter =
(8.5 x=20
      11 inches) paper.</P>
      <P>Arrange the paper in this order: (1) title page; (2) abstract; =

      text; (4) references; (5) tables; and (6) figures and legends.</P>

      reprint requests should be addressed.</P>
      <P>Provide three to six key words.</P>
      <P>For clinical studies, a structured abstract should be =
submitted. It=20
      should contain the following clearly labeled elements: Purpose, =
Subjects=20
      and Methods (or Materials and Methods), Results, and =
Conclusion.</P>
      <P>Provide an abstract with reviews and special articles. Brief=20
      Observations do not have abstracts.</P>
      <P>Abstracts should be no longer than 250 words.</P><A =

      <P>Limit use of acronyms or abbreviations to units of measurement. =

      <P>Measurements of length, height, weight, and volume should be =

      in metric units (meter, kilogram, liter) or their decimal =
multiples.=20
      Temperatures should be given in degrees Celsius. Blood pressures =
should be=20
      given in millimeters of mercury (mm Hg).</P>
      <P>We prefer standard units (e.g., mg/dL); if SI units are =
provided, the=20
      conversion factors should be included. Data should be rounded to =
the units=20
      of measurement.</P>
      <P>Acknowledgments, including grant support, should be placed at =
the end=20

      <P>Cite every reference, figure, and table in numerical order. =

      <P>Follow general arrangement, abbreviations, and punctuation as =

      Uniform Requirements for Manuscripts Submitted to Biomedical =
Journals. For=20
      periodicals, follow INDEX MEDICUS, listing all authors when four =
or fewer;=20
      when five or more, list only first three and add et al.</P>
      <P>Personal communications are discouraged. If included, they must =

      noted parenthetically in the text, dated by month and year, and=20
      accompanied by a written acknowledgment.</P>
      <P>Please note that no periods are used after authors' =

      <P>Start each table at top of new page. Double-space. Do not use =

      <P>Give each table a title. Number them in the order in which they =

      <P>Provide a footnote to each table that identifies all =

      <P>Submit five glossy prints (not originals) of each photograph =

      drawing. Laser quality figures may be submitted. Use one figure =

      print the legend on a separate page.</P>
      <P>Label figures on back by number and first author's name; =
indicate top.=20
      Please note that artwork of published articles will not be =
returned.</P>
      <P>Figures must be of publication quality. Use black ink for all =
charts=20
      (line drawings). Make decimals, broken lines, etc., strong enough =
for=20
      reproduction. Only sans serif fonts (e.g., Arial, Helvetica) =
should be=20
      used; all lettering should be of approximately equal size. Use =
arrows to=20
      designate special features. Submit high-contrast prints for=20
      roentgenographic photographs because some detail is lost in =
reproduction.=20
      Crop photomicrographs to show only the essential field.</P>
      <P>Titles should be included in the figure legend, not in the =
figure.=20
      Number figures in order in which they are mentioned in the text, =
using=20
      arabic numerals (Figure 1, Figure 2, etc.). Identify at the end of =
each=20
      legend all abbreviations used in the figure. Indicate staining =
method and=20
      magnification for all photomicrographs. If figure legends are =
submitted=20
      separately from the figures, they should be listed in order on a =
single=20
